HI Guys
I need a suggestion please help.
I got my Medical request a week before .Along with the mail they have attached a chart , stating i need to complete that attached chart ( sheet ) . That sheet contains applicant name , spouse , children , passport expiry , mailing address , phone number etc . It is named as Update Sheet in the mail .
My question is " do i need to complete the required sheet or not " As i dont have any changes in my application , do i need mention information again or i can send my medical alone to the Immigration Embassy .

I got the mail from london visa office (canada)

Please let me know ?
