i have questions on BACKGROUND/DECLARATION form:
1. in education area, my bachelors(4 y engineerin) done JNTU affiliated college(like adithya engg college). but i got PC(i don't have college name) with name of JNTU and marks memo also with JNTU, but in memo have college name at one area.

which one is i need to write at name of school area and city for my bachelors?
jntu(hyd,india)
aditya engg college(XXXXX, india)

2.)in personal info area ,Your full name written in your native language or script at ? do i need to write 1stname+middilename+lastname
3)Personal history area & Address area, Please provide details about your personal history since the age of 18, or the past 10 years, whichever is most recent.?
i worked in different state in usa , on that case do i need to mention all duration separately? or just duration of worked in usa with recent address.
4)Membership or association with organizations Area, i am member of volunteer in state foodbank in USA, do i need to mention there, if i mention there do i need to submit any proofs for this?

Can anybody take up my query on educational details?

As per other posts here, below are the duration of various schools in India.
Elementary/Primary School : 5 Years
Secondary/High School : 7 Years

What if I appeared for an improvement examination for my Secondary Course. I had passed my Secondary examination in 1st attempt but the marks scored for 2 of the subjects where low so I applied to take those exams to improve the scores, for which I did not attend any institution but did self study to write the exam a year later with the junior batch. During this time I was not allowed to take any course in any educational institute or the new scores would be inapplicable.

Will this make my Secondary/High School duration as 8 years? or that 8th year will be considered a gap year?

In the document it is no where mentioned the submit photocopies of visa stamp
But we have to mention city and country in Address, personal history (IMM5669E), and supplementary information your travels(IMM5562E)

how do CIC verify the address we have mentioned?
