Incorrect Dates sent to CIC. Urgent!!!

UCI is found in the generic form in the 1st page itself.

I wrote for the immigration office Sydney instead of CPP-O and I did not write my Client ID on the form. I also sent the Payment Form which I forgot to complete with my name and address as specified. (I sent the bank draft only). I also realized I should send a copy of my provincial LPN license. I really hope, they will add it to my file.

WHERE DO WE PRINT THE PAYMENT FORM??????? PLS LET ME KNOW

That gives me a hope. I will send the latest work permit paper along with covering letter giving my personal details and also the amended letter. I hope they will attach it. Have you guys talked to CIC customer service about it? What do they say for these mistakes?

I did not bother to call them, A few times I called in past and I got wrong information. Let me know what they say if you get a hold of CIC customer service. Lets keep posted!
