Importer in CBSA B4 form

Hallo again,

I'm going to land in Canada in about two weeks time and I am just starting to fill out b4 and b4a for the CSBA. Are there any restrictions on the importer's name on the form? For example, does it have to be the principal applicant only or can or should the dependants have their own forms?

Also, are you supposed to enter an item's current market value or it's original purchase price, even if purchased a number of years ago?

1. Either Principal applicant for all or each could be the importer on their own, but that seems like an awful lot of work.
2. Current used value.
