Immunization Forms ???

Hi Qorax,

We got our VISAs in February 2010. During our IME, we didn't receive any Immunization Forms from Singapore.

I have sent it to you...

Dear Qorax,

I have checked my children vaccination card and found the following already been applied.

DIPTHERIA
PERTUSSIS (WHOOPING COUGH)
TETANUS
POLIO
MEASLES
B.C.G. (TUBERCULOSIS VACCINE)
HEPATITIS - B

But as per the form that you provided the following are left:

Mumps
Rubella
Haemophilus Influenza B

Since I am moving by June'10 I don't think that I can complete the course of the above.

Will it be possible to give the above vaccines after my arrival in Canada.

My children age is 3 and 2.

Please advice.

Dear RPN,

Visit the doc & 'complete' as much as immunizations as possible before u leave. This is 'coz of 2 reasons:

1. The school might be adamant that your kid/s take the vacs before admitting them-in.
2. Till 3 months u'll be w/out OHIP -paying for the vacs would drain u, they r pretty costly in Canada.

Your Dr's medical records should be sufficient.

I am from the UK...I just produced my sons medical records and they accepted those to prove that he had been immunized.
