Can i choose not to immunize my infant?

What will be the implication legally and while admitting kid into school

Is there any legal form which can be filled and submitted to schools and other forms, that i have chosen not to vaccinate my kid

otherwise u have to complete all immunization in Canada, prior to admission in school. vaccination is must.

I am planning to land in May and my son turned 16 years in last August (2013)-1997 born.
He is eligible to join 12th standard now.
Is Immunization required for him ?
I am asking this because even though all the immunization was taken in childhood we have lost the records.
Can somebody pls answer this?

Is it necessary to get this certificate from home country where he was immunized? Can it be done from another country currently where we are residents? Coming to the point currently we are in middle east but child was immunized in India so can we get the certificate from a doctor here in Middle east or do I have to go to India to get this certificate? Kindly let us know.

Any Doctor who has a license to practice in your city, will suffice
 
Dear Mates,

I have recieved Immunization Record Form for my kids along with PR visas and COPRs. I am planning to land in September/October 2014 in Toronto, Ontario. Kindly share your thoughts if Immunization Record Form are mandatory to be filled and certify by a Doctor in home country to get school entry in Ontario or I can present Immunization Cards issued by Health Auhtority in United Arab Emirates?

Yes, but if there is an incident of any disease in the school, your child will be sent home.
