School board will probably ask for new vaccinations or ask you to take your children out of school if there is an outbreak, such as measles/mumps. Contact the Dr. who did the jabs and ask for a new record.ymanoelpia said:what if we lost our childs immunization record? what shall we do so that we can provide one as part of school requirements.
And how would the new Dr. know which jabs your kids had?ymanoelpia said:If we already lost it and need a new record, do we need to go to the doctor who did the job before or can we just asked any doctor to sign the new immunization record? Is it legal?
