Immigration office / Visa office column missing in latest IMM0008ENU_2D form?

IMM0008ENU_2D form is updated in November 2014. At the last minute before sending those document, I found that there was another IMM0008ENU_2D form updated in Nov 2014. So I quickly filled details in that form again. Now I noticed that there is an important column missing compared to 06-2014 form version. i.e. "4. Immigration Office requested for processing this application" column missing!!!!

Did anyone noticed this? If so, how will they know our visa office requested?

no lvo column in newly updated form. They will asign the lvo for applicants.
