My friend is travelling to Canada for the first time as a visitor. She is worried about what the immigration
officer may ask and if she the customs will check her bag.

I have no idea on this. any information will be helpful to her to prepare.

Normally they ask questions like "what is the purpose of your trip to Canada", "where will you be staying", etc. But they can ask her anything they want to and can certainly seach her bag if they think it's necessary.

What exactly is she worried about? As long as she isn't carrying anything she shouldn't be - there won't be a problem.
 
She is carrying clothes, medicines, and two gift items which are perfumes.

She is worried about the medicines. She is diabetic and is carrying medicines and prescription
for that.

She does not know if she should say that to the immigration officer. I have no idea about medicines
as I have never carried it myself.

She should be fine as long as all of the prescription medication is for herself (and not for someone else). It wouldn't harm to get a letter from her doctor confirming he/she has prescribed this medication to her.

Perfumes have a wide range of prices, as long as they both are under 60CAN each then you should be fine, if not it would be adviced that you declare them, the tax is not much and usually officers would appreciate more that you tell them and let you go (if the amount is too small) than a random search that would lead the officer to believe you are avoiding taxes.
