IMM5713E - Family member Representative

Hello All,

I am applying for my parents(both Mother & Father) visitor visa . I am filling up the forms in CIC on behalf of my father (primary applicant).
One of the supporting document is IMM5713E. But the pdf file is secured and I can't save the content of the file . Only way is to take a printout. How can I upload the IMM5713E document when I can't save it?

Is necessary to sign the imm5713? If yes, bota parts or just the person who gives the representative?
