IMM5669E doubt...

Hi,

Got a doubt while filling IMM5669E.

I stayed in USA for 5 years. During my 5 years stay in went to India 1 time for vacation. Do I need to give this India travel information
under "Personal History" and "Address" section? or can I ignore this vacation travel information? Please advice..

Like –
01-Dec-2004 to 01-June- 2006 - USA address
02-June-2006 to 20-June-2006 – India address (Vacation period) - what activity I have to mention here?
21-June-2006 to 10-Dec-2009 – USA address
