My husband and I sent in our application for family sponsorship in July of 2016 to be processed inland. Yesterday, (October 17, 2016) I received an email from CIC advising me that two forms were incomplete and needed to be redone and sent in via the webform. One of these forms was the IMM5669E form. It looks like what was missed was down at the bottom. It states it needs to be sworn by a Canadian Government Official. I contacted an attorney office in my area to see if they could notarize this form, and was advised that they are unsure they can notarize this form. I was advised to contact IRCC center and see who has to fill out this section so that I can get the form back to CIC within 30 days.

I have been trying to reach CIC all day, but because of a high call volume it will not transfer me to a representative. Does anyone know who needs to sign this section of the form? How am I to get this accomplished so that I can return the forms and my application can continue to be processed. This is becoming highly stressful, and I would appreciate any help available. Thank you!

As stated on the form, that section is not to be filled out unless actually directed to do so with a specific official. Most likely, you missed a month in one of the histories.

Read carefully page 4, they ask donot fill it (only in the presence of cic officer.............may be at the time of interview)

after I validated imm5669, saved and closed, I reopened it, the top right validated changed to No. Why?
