Hello we just received from Paris, that there are some information missing regarding the IMM5669 Schedule A, we have some questions they are saying information missing pertains to question 8 to 12 question 8 which is her personal history has been filled out question 9 to 11 is military service, government positions and any membership or association to an organization which we fill N/A in the first row so we are not sure what there asking for regarding 8-11 and question 12 just says list all addresses where she lived since she was 18 or 10 years ago whichever is recent she just lived at her house which was already listed before so do we just put her house address for question 12?

What would we fill out for 8 - 11 8 is already done so I have no idea what they wanna know and 9-11 doesn't relate to her do we just put N/A or do we have to say that she never took part in any of the following services

In the CIC guide, it has more specific instructions. It looks like "N/A" doesn't cut it:

Question 8

Provide the details of your personal history since the age of 18, or the past 10 years, whichever is most recent.

Start with the most recent information. Under “Activity”, write your occupation or job title if you were working. If you were not working, provide information on what you were doing (for example: unemployed, studying, travelling, retired, in detention, etc.). If you were outside your country of nationality, indicate your status in that country.

Note: Please ensure that you do not leave any gaps in time.

Important information: Failure to account for all time periods will result in a delay in the processing of your application.

Question 9

Indicate the names of all membership or association with organizations you are or were a member of, including:

political organizations,
social organizations,
youth or student organizations,
trade unions, and
professional associations.

If you were not a member of an association or organization, do not write “not applicable”. Rather, write: “I have never been a member of an organization or association”.

Note: Do not use abbreviations. Write the city and country where you were a member.

Question 10

Indicate any government positions you have held in the past such as:

Civil servant;
Judge;
Police officer;
Employee in a security organization;
Etc.

Write “NONE” in the box if you have not been a member of an association or organization.

Include:

the name of the country and the level of jurisdiction (examples: national, regional or municipal),
the name of the department or the branch you worked for, and
activities and/or positions that you held.

Note: Do not use abbreviations.

Question 11

Give complete details about your military or paramilitary service (if applicable). Provide the details of your military or paramilitary service for each of the countries whose armed forces you served in. If you were not in any military or paramilitary service, write “NONE”. Do not leave gaps in time.

Question 12

Indicate the residential addresses where you have lived since your 18th birthday or the past 10 years, whichever is most recent, complete with the postal code. Do not use P.O. box addresses.

Thank you, that was very helpful? Question 8 where it says "Name of Company, Employer, School etc" that last column there most things we have the name for 3 of them there but we have N/A for two should we write N/A or not I am not sure because we aren't sure of the names

This is off thread but when I email Paris back do I start with a new email or can I reply to the existing one that was sent? Also I was sent the IMM5662 travels to complete they sent it to me in a .xps file do I convert that to acrobat and fill it out and convert it back to .xps once it done or does it matter?
