IMM5669 Question 5 - Mother´s last name

Question 5 in form IMM5669 asks for the mother´s "family name at birth".

The instruction guide says "Provide your mother’s personal details including her: last name (surname/family name)...".

My question is: Do I provide my mother´s maiden name, her last name at the time of my birth or her current last name?

Maybe a silly question, but I find the information given on the form and in the instruction guide somewhat contradicting and I just want to make it right.
