IMM5629 #7 Proof of Relationship to Sponsor

Can anybody please help me understand what exactly do i need to provide? The checklist says.
" a written statement attesting to the fact that you and and your sponsor are unable to marry or to live together and explaining why"

Am i gonna make a written statement here like a formal letter?

is it okay to include picture to letter of support of family or friends? like attachment?
