My wife already got PR and living in Cananda from the past 1 year.

Now she is sponsoring me. All documents has been done except IMM5562 Supplementary info where my and my wife`s Travel history should be mentioned and my wife`s travel history is long which do not fits in the form.

My question is:-
1. What is extra sheet looks like any format ?
2. How to attach with this form ?

Mention last 10 years of travel history.
You can photocopy that form and attach.

That means If I cover my travel history in 1 form then I have to go for multiple forms for my wife`s travel history mentioning my travel history again on each form ?

A copy only for your wife's travel history. Not yours. You can mention this in an LOE as well.
