I need a suggestion regarding this form because my spouse could not find his old passport and he forget the specific date when he traveled to Mexico. I need a suggestion. Thanks

Check email for flight confirmation, or maybe pictures than can help you figure out dates, or check your old text messages for dates mentioning Mexico.

I would attach a letter of explanation, because you can't supply pictures of your entry/exit stamps
