Few questions to clarify if I answered these questions right.

IMM5540E questions 4 , 9, 11

This may sound silly but

Question 4
they ask about parents' address.
My father is deceased, so do I put "Deceased" in address box or Not Applicable?

Q: Are you a naturalized Canadian or permanent resident of Canada?
A: Yes, I am naturalized

Q: On what date did you become permanent resident?
A: I am not sure.. is this same as receiving Landed Immigrant status?

Q: Which Canadian visa office issued immigrant visa or confirmation of permanent residence?
A: not sure, on Landed Immigrant page says ETOBICOKE, is this what I put in here?

Q: Are you currently living in Canada? Since when:
A: From 1995 till 2007 I lived in Canada, then left for Germany 2007-2010, and I am back in Canada since Feb 2010.

** Which date do i put here 1995 or 2010

I know lots of questions but I tried to make it as clear as possible. Thank you in advance to whoever helps me out here.
 
Can someone at least answer mem please , is landed immigrant same as permanent resident .
 
yes. When you landed in Canada on your immigrant visa that is when you became a permanent resident.

Either would be OK. I'd put 'deceased' just to be sure they see it.

Yes, it's the same date.
Q: Which Canadian visa office issued immigrant visa or confirmation of permanent residence?
A: not sure, on Landed Immigrant page says ETOBICOKE, is this what I put in here?
Put the visa office that issued the visa - that is, the visa office in the country where you applied from. For example, if you are German and applied through Berlin, put Berlin. There is enough room on the form, so you can also put "Etobicoke" - you can write that your landed immigrant form was issued at Etobicoke.

Put 2010. There are other places in the forms where you have to list all the places you have lived, so they will know about your earlier residence in Canada.
