IMM5540 - question

Hi,

I am in the midst of filling out IMM5540 - Sponsor Questionnaire and need clarification of how you guys might have answered for the following:

#9 Are you naturalised Canadian or PR of Canada.
I answer Yes (was naturalised ages ago) but the box next to Yes is asking "on what date did you become a Permanent Residence?
A little confused here. Do I put the date I was naturalised as a Canadian or the date my family moved to Canada as PR many many years ago?

#11 When do you plan to return to Canada to resume residency?
Do I have to put a date here? It is obviously pending approval of my husband's PR application. Did most of you put an estimate date (taking into account the processing time on the CIC website)? any advise?
Also, what details should be given to show plans to re-establish? What if we don't have a job offer and planning on going to look for one when we land? if i send out emails to my previous managers is that part of plans to re-establish? I doubt either of us will have a job offer before we move and i also doubt that we will find a place to rent before we move. We plan to maybe stay with a friend or an aunt for the first couple of weeks.
Any tips or ideas on how to tackle this answer?

For #9, enter the date of original PR. It's a prerequisite to becoming a Canadian citizen anyway..

For #11, enter a reasonable estimate. We applied in February 2013 and we entered 1st January 2014 in that field.

Provide whatever evidence that you can regarding steps being taken to relocate back. It's an open ended question, and CIC just need to see a reasonable timeline, supported by evidence of activities such as researching jobs, accommodation, schools (if applicable). Offers of practical support from family and friends inside Canada is very helpful, such as accommodation...

On the same form, Question #8 asks - Have you resided in countries other than Canada?
And if yes, addresses, phone numbers and period of residence at each address to be given.

Sponsor was born outside of Canada and lived in birth country till 10 so not 100% sure of the addresses and periods at each address...definitely don't know phone numbers

What should be filled out for this question?

Sorry but I also am looking for help here.

On the application forms when it asks what kind of Canadian citizen:
Canadian Citizen by Birth
Canadian Citizen by Descent (Citizenship through parent)
Naturalized Canadian citizen (after immigrating to Canada)
Permanent Resident

I've tried to look it up but am having trouble determining what citizen I am.
My parents and I were born in Taiwan. However, we immigrated to Canada when I was only ~1-2 years old, lived here my entire life after that and have Canadian citizenship card with my child picture on it.

My husband is sponsoring me, and he also had trouble answering these questions...he came to Canada when he was very small...so he called CIC because he was unsure of the date and the question...and they were kind enough to provide him with the answers :)
