#7 - Says that the dates i started and ended dd/mm/yyyy .. and the monthly salary .. but how am I supposed to remember that? And my monthly salary varied since I was only working full time
 
Write "unknown" if you don't remember the day. But do your best to remember at least the month and year. Take an average of your monthly salary for the time you worked there. For example, if you earned $2000 one month and $4000 the next month, the average would be $3000 and that's what you would write.
 
i dont remember having to state salary on my form... :o
but if you have done the paperwork for a temporary visitor visa, they ask you that as well, so if you have a copy of that then you will know what to put.

Okay good, because I do remember the months and years. But I don't remember how much I've made each month, can it be like an estimate average?

I called about this, and CIC agents said that they just want an estimate. However, as you may be aware, income for sponsoring of spouses (I am assuming this is who you are sponsoring) is not a determining factor for approval so long as your spouse has no children who have children of their own. At least that's what the CIC agent told me.
