Hello experts, can you help with this question on my application form?

My wife and I got our PR 10 years ago and she is now a citizen. As I had to take care of my father, I had not been able to renew my PR card. In 2015 I renounced my PR as I urgently needed a visitor visa to see my daughter's graduation. At this time, I am looking to reapply for PR under Family Class and will be sponsored by my wife.

Based on my situation, on IMM5533 for question "Information about Previous Examination (to be completed by the
sponsor)", would this be YES for me?

I don't see the question you are asking about, what number is it?

it is the first question in IMM5533, before Part A of the form.
The answer is yes and no. I don't know under what program the 2 of you first got your PR through but I suggest you answer YES, you were declared because you weren't a non-accompanying spouse. You will have to include an explanation in your LOE of your past situation. I think the important part is that you were declared as opposed to being not declared which would make you ineligible of being sponsored in the future. I am sure you were planning to detail all your past PR history anyways. So in your LOE make a reference to that particular question and give an explanation.
