IMM5490 question 30 and 31

Please I ask help, can you help me, please is so so urgent.
In the Form IMM 5490:
IMM5490 question 30 and 31


1. Why when I press YES in the question 31 (about my consent to disclose), automatically it made delete the previous question, number 30?

In the question 30 I fill the quantity of the the options: How much/ How often.

but I don't know why it is removed, when I fill the question 31.


I don't know why.. somebody know why and is it correct?

I know this has happen to me as well.. there is something wrong in this form.. but don't worry what I did was.. I said yes to #31 and then wrote it with black pen on #30 how much and how often. as it is not the form which we need to validate so you can also fill this up with black pen.

That is a bug sometimes.
Choose an answer for question no. 30 and after you print the form, you tick the "yes" box for question no.31 by hand. Don't forget to sign and date also, you need to answer question no.31.
