Hello,

Can anyone advise me on IMM5406E Additional Family Information Form. Do you have to fill out your parents name if they are not alive? Thank you for your feedback!

Yes, please fill out your parents names even if they are deceased. In the Present Address column please put deceased/expired.

Generally, please fill out everything in your forms otherwise they will consider it incomplete. If you want to attach additional information, put it on a blank sheet of paper as an addendum. Finally, ensure your application reaches by the end of this month. I'm not sure if they would consider it otherwise.

Could you please advise if i forgot to attach IMM5406E for my spouse and my application has not reached CIO yet, may I send it additionally with a cover letter so my application is considered or it will be returned as incomplete ?

Such frustration after all the hard work, your advises will help a lot.

I would like to change what I said before.

I just checked my application where I have one parent deceased. In Marital Status I put Deceased (and Widowed for the other parent). In the Present Address for the deceased parent, I put Not Applicable.

If both of your parents are not alive, put both the Marital Status as Deceased and the Present Address as Not Applicable.

Please don't hijack someone else's thread.

Anyway, yes your application will get rejected because it is incomplete. Your best option would be to send the missing documents along with a cover letter pleading them to attach them to the original application. Please include the tracking number of the original application in your cover letter so that they can merge the two. This option is not guaranteed to work, but it is better than not doing anything. On the other hand a few people on the forum have sent additional documents a while later and got the PER. So you can try this route.

Please help me with one more question. Do you fill out the names in your native language on the same paper or use different sheet of paper for native language spelling. Also what should I do if I cannot write in Armenian in this pdf form? should I print and write by hand?

Actually, I did not have this problem, so unfortunately, I cannot guide you.

However generally, in case of all forms, you can attach additional documentation i.e. in the form of a different sheet of paper. I think this is what you should do. You can also write it by hand, but there does no seem to be a lot of space, after you write the English names which are required.

Put the English names on the form. This is required. Also put a note (even by hand after it is printed) that you have an Addendum attached.

Then on a sheet of paper put the additional information that you wanted to add to the form but could not. If you could type in Armenian, that would be preferable to hand-writing it. But you can also hand write it, if you want.

Finally, they generally ask that you do not staple the forms etc when submitting it. (I think it is because it is easier to scan without the staples.) So to ensure that someone can match this Addendum to the form, put your First name, Last Name, Date of Birth, and the Form name that you would like this to be attached to. (In this case Additional Family Information) That way if it gets misplaced, someone can match it up later.

Also for the own copy of my husband, as an accompanying spouse, should he put his name as the applicant and then my name as his spouse on this confusing form?

Yes irina123, in your husbands copy of the Additional Family Information form, he would be the Applicant and you would be his spouse. (I am assuming that he is applying together with you i.e. he would accompany you to Canada if your application gets approved.)

Basically, for all forms:

You -> Principal Applicant
Husband -> Applicant

I just want to add that I hope you send your application by the end of this month. If you have some time please look at the forums for the reasons why other people got their application rejected. This way you can avoid making the same mistakes.

First look at the spreadsheet. Right at the bottom there are people whose application got rejected. Please read the comments in the Remarks column.

Hey, Im sorry for following up with such an old thread, but I currently received this ADR for family info. right now and I have a few questions I was hoping someone could help me out with.
1. For the spaces where there were no applicants, eg. (I'm single) so for spouse I have mentioned it as "not applicable" only at the place for name. For the rest of the spaces at that section eg. present address of spouse etc. I've left it blank, as I've already mentioned not applicable under the name. I just wanted some clarity, to avoid any case of incompletion.
2. The form says that I would need to fill it in english and my native language. Though I'm a citizen of India, my name's not of indian origin so would that still apply? I mean would I have to submit the form in Hindi again?
3. How should I have it submitted? I would have to fill the spreadsheet form on the computer, print it out, sign it physically with pen, and then have it scanned and attached. Correct?
Please do respond quickly, as I have just 7 days to revert back. Thanks.
