- Look for the location of the file/form downloaded. Double-click on it to open it in Adobe Acrobat or any PDF viewer.
What you are doing by clicking on the link is opening the form within the browser. Firefox and Chrome needs to have updated extensions (adobe) in the browser to have the PDF document opened inside the web browser. The easiest workaround is to download the form, and open the file outside of the web browser.

The link is dead, and I cant download anything from here. I thought this might be the browser issue, and tried bunch of different ones but no luck!
I used another IMM5257e from dated 2013, but when tried to upload to the website it gave me an error, that this is an old version and not accepted. Any chance any of you guys have a more recent form that can email to me and help me get over with this madness!
The website now only accepts the forms updated on March 2014 and later.
