Hi I have received very similar email asking for:
- Police Certificate: From each country/state where the residence period has been six months or longer since the age of 18, provide an original police clearance certificate. Please submit an updated ORIGINAL police certificate from Croatia. PHOTOCOPIES ARE NOT ACCEPTED.
Strange thing is, that I have submitted original one already with my application package...HELP!

- Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please submit a new Schedule A with details for Personal History from May 2013-Oct 2013. This must be received at this office by: 2016/04/29.
I am not sure if they ask for the whole document to be sent again with all dates or only that part, because it is so hard to remember all the dates AGAIN! I dont know why they ask for it again!

Did anyone had similar email request?

You should send a Case Specific Enquiry, alerting CIC that you've already sent the PCC, while you start the process to get another one (in case CIC doesn't change their mind about needing this).

You can also mention the IMM 5669 form that you've already provided, but it looks like they have a specific question regarding those dates. You hopefully made copies of your original form before you sent off the package, so just review that copy and see if you can fix whatever the problem might be. You should complete the newest version of that form again and send it to them ASAP (with delivery confirmation).

Sometimes it's a mistake on their part, but even if it is you have limited time to give them what they want.

No, that question is pretty clear, it says "Have you ever", so at any point in your life, from birth to now.

They should really write that you need to account for EVERY month on the form itself. 'Leave no gaps in time' is far too vague.

By Jan end Immigration requested to submit medical, Police clearance and updates schedule A form before feb 29. And i did medical last month and submitted all request documents on feb29. So i need know what will be the next? is they will ask any other document or is i hav any chance of rejection again?

Can i know y they asked us to submitted schedule A form with police clearance and medical?

And My ecas changed to medical received at the end of feb cause i did medical feb 1st week.

How long does they will take to reply me back? after i submitted all requested documents( which is police clearance and schedule A form submitted on feb 29). Its been 3 weeeks now...i am worrying too much. please someone help me on this?

Also, I have overlapping dates. Example:

05-2009 to 12/2011 - employed
08-2006 to 01/2010 - education
07-2007 to 04-2008 - internship
04-2008 to 04-2009 - internship
04-2009 to 05-2009 - internship

so as you see above, I was studying as well as doing internship...Is This is okay??

What if you put your past 10 years activity? is that ok?
not all the way from your 18 yrs old.

Because as per instructions, since the age of 18 or the past 10 years, which ever is most recent.
i chose in the past 10 years. is that correct? 2005-2016 based in my employment.
