I am not sure if they ask for the whole document to be sent again with all dates or only that part, because it is so hard to remember all the dates AGAIN! I dont know why they ask for it again!

You should send a Case Specific Enquiry, alerting CIC that you've already sent the PCC, while you start the process to get another one (in case CIC doesn't change their mind about needing this).
You can also mention the IMM 5669 form that you've already provided, but it looks like they have a specific question regarding those dates. You hopefully made copies of your original form before you sent off the package, so just review that copy and see if you can fix whatever the problem might be. You should complete the newest version of that form again and send it to them ASAP (with delivery confirmation).
Sometimes it's a mistake on their part, but even if it is you have limited time to give them what they want.
