- Should I put the City / Country of my school / employer or should I put the city /country where I lived during the year ?

I am not sure which is right ? Thank you for your help.

You should put the city where the activity took place. For example:

Unemployed - your city of residence
Employed - the city of your office or employer
Study - the city of your university
Travel - the city where you travelled

I asked this question because I worked remotely for my employer, and thus I lived in different cities during my employment. Do you think I need to provide a letter of explanation for difference in address of my residence and employer ?

Yes, I would be honest about where the company is located and record that city in personal history while recording your residence properly in address history. You should definitely explain the discrepancy in LOE since VO will wonder.
