IMM 5669 HELP NEEDED

when filling up the IMM5669 form, it asks to fill 10 year activity history.

Now, there are only 5 slots available to fill the personal history. If I break down my history, I would need more than 5 slots.

What do we do in this case?

Couldn't you attach a new paper (stating your information and the part of your previous activities that didn't fit in the form?)

You can Mention "Refer to Annex I attached" in one of the rows and create as many rows of data in a supplementary sheet.

Supplementary sheet will be a piece of blank paper and all the history will be handwritten? or there is another form?

Yes a blank paper, but make sure you will have your name, document and the question number you are answering on it.

Just a Blank paper. Create a similar looking form (similar to IMM5669) in MS Word with ur name.. Fill additional data.. Print..sign..scan and attach it with the IMM Form and transmit.

Also be sure they want the 10-year history. We filled out the whole Schedule A, but later realized they had specifically asked for my partner's statutory questions and signature - we didn't need to do the rest.

They said to specifically fill the question 6 of the form.

I guess everythings that comes with that form needs to be filled including travel, work and personal history.

Isn't question 6 the statutory questions? That's what my partner was told to answer, and then the note said they needed an original signature and date as well. It was very specific.

I just checked - it is the statutory part. Of course I would fill out the name section, but I believe all they need from you is the answer to the statutory questions and, if they asked for it, a signature. If you feel better doing the whole thing it's up to you, but they have been quite clear with almost everyone as to which answers they want.
