I am a Canadian citizen and am the only child of my mother, who I am sponsoring to come to Canada. My mother was never married. I won the application lottery and filled out the entire online application package for family class sponsorship on behalf of my mother. To the best of my knowledge I entered all family members on the forms. However, since the application was submitted over 3 months ago, I cannot go back to see exactly what I filled out on the forms. Today I received a letter from IRCC that confuses me. In the letter they stated the following:
My mother has filled out and signed IMM 5406 again. Should I just email this back as a single form to IRCC and ask them if there was something that I did wrong or if I must also fill out and sign another copy of the form myself even though I do not qualify as a dependent myself under IRCC's definition? If I email them the form, will IRCC contact me back to tell me if I sent it incorrectly or correctly? How will I know if my application is proceeding or if they will just throw it out if I get something wrong this time?
Please help, I really need to get this right.

As far as I know, I filled out the IMM 5406 form correctly, but I cannot go back to check. I am 36 years old, and financially independent with a common-law partner, so I would not qualify as a dependent, so I believe that only one copy of this form has to be filled out by my mother. But this email from IRCC makes me question whether I have to fill out a copy of it for myself? Also, without the ability to review my submission, I do not know if they sent this because there was possibly some missing or incorrect information on the form that I filled online.Your application does not meet the requirements for processing as the following item is missing or incomplete:
- IMM 5406 (for Principal Applicant): The Additional Family Information (IMM 5406) provided was not completed fully. Please provide a current, completed, signed and dated Additional Family Information (IMM 5406) for the Principal Applicant and all dependents aged 18 and over, whether accompanying or not. Each individual must sign their own copy of this form by hand.
Sections A, B and C must be fully completed. The Name, Marital Status, Relationship to applicant, Date of birth, Country of birth, and Address must be filled. The street number, street name, city, postal/zip code and country are mandatory for Canada and USA addresses. All other countries must provide the City, Province/State/Parish and Country.
If any information is not known, please enter in “Unknown”. If you are unable to provide any of the information listed above for any individual listed in section A, B and C, you must provide an explanation. You must submit the most recent version of this form.
