IMM 5604 - Both the parents are PR holders

I am in canada with canadian PR . My wife is in India and has canadian PR. My daughter born in india and recently I sponsored her for canadian PR. But my applicaiton got returned satting that IMM 5604 to be filled. How this can be applicable to me? whats the next process ?

Hey Canuck. I am requested to submit an IMM 5604 declaration, my ex-husband is preparing it back in my country of origin. The notary public stamped it in Arabic which needs to be translated. Does he have also to write an attestation stating he verified and witnessed the signature?
This has been mentioned already in the form so will the notary signature and professional stamp be enough? or should he add the attestation which states that ( before me, I ....... then title the parent's name has signed this form after verifying his Id and to authenticate it I have placed my stamp on this document and dated mm/dd/yyy)
