Imm 5562

Hi there, I need help on filling up the imm 5562 form for my Home Support Worker Pilot application.

10 yrs ago (2011) I was still in Philippines (country of origin) then came to Singapore (work permit residence) from 2012-2017, 5 years. Q1= do I have to include Singapore as my travel?
Q2 = do I have to include my vacation travel back to Philippines while I was working in Singapore in that span of 5yrs)

2017 April to May, 1 month I stayed in Philippines again.

2017 May, I had Au Pair Visa to Norway, and I stayed in Oslo for 4 months (May - Sept 2017)

Late Sept 2017 up to present, I am back here in Singapore for work permit residence again.
Im only sure about those short tourism travels out of singapore I had.
Please let me know what to include in all the travels and what not. Thank you.

Hi Po yes you have to declare each and every trip made last 10 years . Even it was one day trip . U can go through your old passport for reference.

How did you sign 5562 form? There is no space for signature!
