IMM 5481

Hi all, we just filled up the IMM 5481 : sponsor's evaluation form and we saw that there is no place to sign at the end of the form. It's 2 pages right? May be we are thinking too much but we just want to be on a safer side . So please advice....did we miss something or is the form supposed to be 2 pages and need not be signed.

And another thing is we are going to pay the fees online so for that we don't need to have a form mentioned on the checklist right? IMM 5401. We just need to submit the receipt that we print out after paying the fees online? Please confirm on this. Thank you so much for all the help thus far. THIS FORUM IS GREAT!!!!

That is correct. IMM 5481 does not need to be signed.
You are also correct about not having an IMM 5401. The online receipt is the alternative to it.

Copy 2 of your Recetpt (lMM 5401), stamped by the financial institution where you pay tne fees or, if you are a
Canadian citizen and are residing abroad, an international bank draft or money order, in accordance with the
instructions given in the Guide to sponsorship, or if you are paying using the lnternet, copy 2 properly completed of the Receipt you print from our website.
