Where do I get the supplementary form IMM 5455 if I am outside of Canada?

Why would you need it? You haven't "landed" according to your earlier posts, so you DON"T apply for your PR card, it is processed at the "landing" no additional forms are required.

Thanks a lot!!!! SO how do I apply for PR card? It says that "All newly arriving immigrants will receive the card as part of the immigration process" What does this mean? And do I need anything else or fill out any forms ahead other than my Confirmation of PR, Passport with Visa, Goods Declarations forms?

You don't have to do a thing, it will all be handled as part of the landing process, and you don't need any forms.
