IMM 5406

Hi everyone,

On IMM 5406 (Additional Family Information) as Stated:
Completed, dated and signed by everyone in the following list:
. the principal applicant
. spouse (whether they are accompanying or not)
. each dependent child over the age of 18 (whether accompanying or not)

Are they (my spouse and my dependent child over 18 years old) going to fill out another IMM 5406? Or, sign the IMM 5406 which I filled out. If my dependent child over 18 years old will fill out another Additional Family Information form, what form will she use? There is no column in IMM 5406 for Dependent child over 18 years old. I am referring to the upper portion of the form..

Please help....

This is pretty straightforward. Print out as many forms as there are family members. Each person fills out their own form and puts their own name under "Applicant", followed by details of their family members.

So ... in my form I have my name under "Applicant" - because I am the principal applicant.
In my wife's form I am the Applicant and she is the common-law partner or she is the Applicant and I am the common law partner.

For me it is a little bit confusing because I don't understand if Applicant means the person who completes the file, or the Principal Applicant in the immigration process.

When your wife fills the form, her name would come in 'Applicant' field and your name in 'Partner' field and when you fill the form, it's your name in the 'Applicant' field and her name in 'Partner' field..

I understand that IMM 5406 form needs to be filled by "primary applicant". In my case primary applicant is my wife and we have no dependent children and I am the sponsor for her. The confusing part is that IMM 5406 is asking applicant's spouse to fill and sign this form also.

Does that mean I (sponsor) also have to fill out another IMM 5406 form? Please someone confirm. Thanks!
