If a parent is deceased, still list the parent and give the date of birth. You can then state he/she is deceased - I'd put it in the 'present address' section..
As for the medical report, if the receptionist gave you the wrong form, you should complain to CIC about the doctor - plus talking to the doctor or receptionist as well to see if you can get the situation resolved.

hubby already did that the receptionist said it the corect one and give CIC their number to call.from what I can gather it is the copy 1 we received which the receptionist put acarbon paper in between the original,copy 1 and 2.Copy 1 was the one she gave us and the carbon writing was appearing well, so she wrote it in ink which kinda make it like fake not original that what Im thinking and why the photo was returned it was the same specification.I m a bit surprised .I knew I missed the form Imm 5406 when someone else in the forum received the back his application for same reason then I realised why mine was taking time to be approved.Anyways hope its going to be ok.

by the way if I want to complain for the receptionist not doing it as supposed how do i report this only to prevent others from having same problem.Im not reporting the receptionist its just trying to prevent someone else commiting the same mistake..we learn from others mistake and problem.
