Can anyone help me to fill the Imm 5406,im a bit confused who is the applicant ans the spouse and the brothers and sisters name does it have to be for both partners or names of brothers and sisters only for the one to be sponsored.Please help me as I did not include this form and my application has been returned and more waiting time :( Driving me crazy

Only the applicant need to fill this out. He/she must include parents, spouse (the sponsor), any children, and any brothers and sisters. This doesn't mean they're accompanying, just ANY AND ALL family members must be included on this form.

By applicant im a bit confused ,or its the stress of the returned application which is making me unable to think as even the medical report form was sent back and asking me for original but it was the original my hubby eceived from the medical practitioner in london or Im wondering if the receptionist did a mistake by handling us the copy...im kinda freaking out with the date limitApplicant u mean my hubby has to fill the form as Im sponsoring him.This is driving me nuts

This form should be completed by the applicant (the sponsored person). So all the imformation should be concerned with the sponsored person (parents, siblings). For sections A, don't forget to use both English and your native language. Good luck.

hey thanks alot .Returning application makes it worst .The waiting time is so stressing and having all the application back is so stressing.Thanks Guys.

This your husband should fill out the form. The medical one is the copy 2. If you didn't make photocopy, I guess that should be it.

Totally understand that. Relax and clear your mind. Have the required documents sent before the deadline. Don't give yourself too much stress.

Yea the receptionist gave us the copy 1 .we did not make any copy .I wonder why they said they want the original and even they asked to write the name and date of birth of the photos ...Im so stressed with that to send it back until 19 december just received the returned application.Besides my hubby is in london .trying to get this papers from UK to Toronto might take time .I was dreading to receive the application returned back now its happened its the waiting time that has increased :(
seems Im going to have a hard time with this application

by the way if one of the parent is dead what should I write in the parent name and date of birth .please help nd thank you

Don't be sad. I suggest that your husband should go to the place where he was examed and ask them for the copy 2. See if they can find it. If the answer is negative, he should do it again and mail the form and copy 2 to you ASAP.

I am not sure about that, but in IMM5669, it asks to give the date if the parent is deceased. Maybe you can just leave it out or make it clear in the same line that he/she is deceased.

yeap going to ask him to do that but y should we pay for a receptionist fault and do the exam and pay the fees all over again .Do you see what I mean?Cause the medical cost around 400 pounds.Anyways who to blame?Im just looking at the form ,Im trying to find any reason ,I can see the receptionist has written the address with a pen and the others was filled by a carbon paper.I think this makes it look like copies but I never thought this would be an issue otherwise I would have told the receptionist to do it well .Anyways hope this dont happen to others.Im sharing this so as if it happens to anyone please do the necessary change before your application get returned .Believe me folk its not easy when application get returned /loads more stresses...

It's not your fault. Maybe your husband should talk to them since they gave your husband the wrong one. Yes, totally agree that it is expensive for some simple and basic medical check. I forgot what was on the copy, but I know what I should get. I checked if it was copy 2 when she gave it to me. On the paper, it printed clearly that it is copy 1 or copy 2.
