IMM-5406: Additional Family Information.

Hi all,

I am a PA from India.
Following are my queries in the IMM 5406 form:

1. The instructions in the beginning of the form state:
Complete ALL names in English and in your native language.
Does that mean, i write the names in English and Hindi (Which is India's national language) ?
But would "Native" language mean any regional language (Multi-lingual and my mother tongue is a language called Malayalam) ?


2. We do not have children. So section 2 of the form is not applicable for us.
So do I write "Not Applicable" in all the columns or just in the name field ?
Similarly i have one sibling, and i entered the details which populated one row in the section.
In this case, do I write "Not Applicable" in remaining sections or leave it blank ?


3. As per the form's heading / instruction on the form, the term to indicate empty section is "Not Applicable".
But the Complete guide link (canada.ca) states use "N/A".
Which of these is apt ?

1. Native language means the language that is your mother tongue and is presumably the language used to give the person their names. If that is Malayalam, then use it. If documents carry a name in Hindi instead, you may wish to use Hindi. I don't know if Malayalam and Hindi use the same written language. If they do, just use that.

2. I would write "Not Applicable" across the entire first row, if possible. If there isn't enough room, use "N/A". It should be fine to write N/A in just the first field.

3. Both are fine. Use "Not Applicable" if there is enough room, and 'N/A' if there isn't.
