IMM 5406 - Additional Family Info, CIC asking for the form again, please help.

Hi,

CIC has requested this form again from the Principal Applicant (my wife), seems we cannot get it right. They asked for it back on March 29th.

Their response was an Incomplete form due to:

For the Additional Family Information (IMM5406) form you must complete the required fields for all applicable family members.

So we reviewed the form, seemed my wife did not put "N/A" where it asked for SECTION B - CHILDREN **she has no children**, instead she left this section empty.
We put N/A for the Entire Name Row for SECTION B - CHILDREN. Left everything else the same, printed the form, signed it, put today's date on it, scanned it and Sent it via EMAIL.


Now is Jun 1st, and they sent her an email stating the form is still wrong, with the exact same message :-X

So I'm reviewing the form, Section by Section to see whats wrong.

Section A
-Applicant: her info, as she's the principal applicant.
-Spouse: my info, as I'm the Spouce and Sponsor.
-Mother: her mother info
-No boxes left empty here.

Section B - CHILDREN
-In the Name Column, we Put, N/A for every row.
-All other boxes empty

Section C
-She has two "half-sisters", same father, different mother.
-Filled info for each sister.

What else could be wrong?
-I've looked it over, cannot find anything, all fields are filled, except some of the Section B boxes as she has no Children, maybe I need to put N/A in every box of the Section B ?
-I've read mutilple opinions on this, but seems for Spousal Sponsorship Only the principal Applicant needs to fill this form, or do I have to fill one too as the Sponsor?
-Put "Not Applicable" instead of "N/A" as the form states at the top?

Any help or suggestions with this form I would greatly appreciate it, if you think I'm filling it right/wrong please let me know, I dont want to 2 months again, for them to tell me I filled it wrong.

Section A - did you also enter her father's info? If not, enter it. If you need more space, attach a separate paper, write IMM 5406 on it and the question number then provide the info.

Section B - write "None" in the 'name' column for every row instead of "N/A". "N/A" is not the proper answer here because this question applies to everyone - CIC wants to know with certainly whether she has children or not. I think this is where the issue is.

I've reread the description of IMM 5406. The first sentence states: The principal applicant, their spouse or common-law partner (if applicable), and all dependent children 18 years of age or older must complete their own copy of this form.

Maybe you should fill one as spouse.

The OP is the sponsor and does not need to fill it out. Only the person applying for a PR visa fills it.

This form is generic and was set up to be used in all types on sponsorships. If the OP was sponsoring his parents (for example), then his mom and dad will need to complete their own copy of this form.

Also, if you look at the sponsorship package, this form is only listed under the "applicant's" part.

If your wife has no children,all you have to fill out is the principal applicant section and the rest should be NA
You don't have to put your name in the spouse field as you are the sponsor

Actually, his wife has to put his name in the "spouse field" because he is her husband and CIC is collecting information about *her* family. This is how I completed the form and had no problems.

You have to put her father's information in the form, not just her mother's. If her father is deceased, put his info in, and 'deceased' as his address. If you don't know his address or other info, fill in what you can and attach a separate sheet of paper with an explanation.

Her father's info is also there, just forgot to mention.

After reviewing, seems my wife forgot to put the Country of residence in one of her sister's info (address). :-\

I will put Not Applicable in the name column where they ask about her children as she has none.

Just looking for clarification that only the principal applicant fills out this form

At the top of the form it reads:
"The principal applicant, their spouse or common-law partner (if applicable), and all dependent children 18 years of age or older must complete their own copy of this form. Complete ALL names in English and in your native language (for example, Arabic, Cyrillic, Chinese, Chinese commercial/telegraphic code, Korean, or Japanese characters). If additional space is required attach a separate sheet. If a section does not apply to you, write "Not applicable"."

I took this to mean that both applicant and sponsor need to fill one out - no?

Not the sponsor as he/she is not immigrating to Canada. As someone mentioned, this is a generic form used in other programs ( temporary resident visa, PNP, study permit etc... ) and applies if the applicant is married to another foreign national and both are applying to come to Canada . In this cases , both foreign mationals must fill out the form.
