hi can anyone tell me if you can get the IMM 5401 forum anywhere in Toronto or is it only through mail?

for payment of the BOWP if you don't do it online and use a financial institution

Looks like the right link.
Check: http://www.cic.gc.ca/english/information/fees/result.asp?lob=wp&canada=inside&cat=Temporary+Residence
At the bottom of the page it says 'order a receipt...' so you have to order it
and CIC will mail it to you I guess.

This form is not available online. You must order an original receipt by mail.
