Imm 5257 or imm 1295 form for work permit application

HI all,

I have got a nomination from ontario province under the OINP category of skiled workers with job offer.

I have created my profile with all the answers, but in the application form it is showing me imm 5257e, i have tried changing some of the my answers, but i still get that form, also int he documents it is showing correctly to upload workpermit documents etc. etc.
MY question is that for a 2 year work permit (lmia exempt) do i need to fill the IMM5257 or imm1295.

If it is the latter, why is my profile showing me imm5257
