hello. my husband had his medical done last nov 6th and he was given the medical report section a IMM 1017. he did this in philippines. now i checked the appendix a doc checklist and its asking for the copy 1 client biodata and summary imm 1017, is it the same one that he was given last nov 6?

Yes COPY1 client biodata is the APPENDIX C while the summary 1017 is the COPY2

Hi,

My husband went today to Al Habib Medical Center in Riyadh Saudi Arabia, for a Medical exam.But unfortunately, It was'nt done because the receptionist in the hospital asked him the IMM 1017 form?WHICH WEBSITE CAN WE PRINT THIS FORM? It's none on the CIC website either.
