I'm baffled by IMM 1017

Hello,

A few questions about IMM 1017, the medical examination that is required for getting a permanent resident visa:

1. Which IMM 1017 is it when you go? There seem to be four different types.
2. What do you need to bring to the medical exam?
3. I'm Canadian, currently in Canada. My wife and two children (my son is 8, my daughter 4, both born in Japan) are currently in Japan. Do all three need a medical exam?

1. If you were a Canadian citizen at the time of the children's birth, then they are citizens (unless you received your citizenship through descent) so you apply for proof of citizenship for them. They are not included in the sponsorship, just listed on your spouse's IMM 5406.
2. You don't need the IMM 1017, the DMP has the forms, your spouse tells the DMP that is a spousal application.
3. Check the checklist for her country of residence, to determine the no. of pictures required. She will also need to take her passport.
