If I changed my legal given name, How to answer "have you ever used any other name?" of imm5710e for PGWP?

I changed my legal Given Name in my home country before coming to Canada and am currently working on the "imm5710e" document for PGWP application.

In this situation, in the question "have you ever used any other name?" in this document, should I check "YES" and enter my old Given Name? And should I also send a document from my home country showing that I changed my name?

I would really appreciate your help.

Have to do both, check "YES" and mention old name and also attach the document
Why to wait for ADR and longer wait time in file processing?
