Can he apply for study visa if we get offer letter from college with english prerequisite?

It's not a good idea. Strongly recommend rewriting IELTS and improving your score so that you have at least 5.5 in each of the four categories.

If any college offers them admission with this english result then you can be sure they are just a useless degree mill. How will your cousin cope with study in Canada with 4.5 in reading? They need to retake the exam and achieve a minimum of 6 in each band.

Can he give Celpip from India ? Or do they consider PT ?

Even if the applicant gets through. How do you think he/she will cope with studies with such poor language skills? Its just setting yourself up for failure. Your cousin should improve language skills and then reappear the test. Overall it will be a win either ways.

I think India only accepts IELTS or CELPIP for SDS stream. For non-SDS they only accept IELTS academic. There's no way your cousin is getting a visa with that result, even if they got an admission from a college. They need to retake and improve their marks.
 
Don't waste your time with that IELTS score, you'll be rejected.. Give it again and secure at least 6 bands in all modules..
