Sorry I am not aware of any format.Is there any specific format in which proof of funds need to submitted? I just put all documents one after other in a pdf. The first page explains each document in detail.

GIC wasn't necessary in your case. But do attach it again, fresh bank statements, ITR if any, updated status of fixed deposits and any other assets that you can show. You should at least have 1 year tuition fee + 10000 (already covered in GIC) and textbook + other expenses mentioned on LOA. Its better to show 2nd year funds as well if you have them.I submitted the following:
1. Savings from my own account.
2. GIC of 10000CAD transferred to CIBC, Canada.
3. 3 fixed deposits.
