Medical Instructions - is just for me or all my family (mother, father, brother, sister) my brother is 15, my sister is 20
Passport Photocopy - just my passport photocopy or all my family
and Citizenship Certificate - just my citizenship or and family

I don't think you can include your parents, brother and sister in this application. But once you became a permenant resident/citizen of Canada, you can sponsor them to come here under family sponsorship if you meet the LICO requirement.

You cannot include your family members excluding your wife or kids. You can call them later on, when you reach Canada. Only your documents are required in this case.
