I need help in filling Application To Change Conditions..., for Open Work Permit

Hi All,

I am applying for a Work Permit under the Bridging Open Work Permit. I am planning also not to go back to my current employer once I get my Open Work Permit. I need help in filling up my application form as to what I am going to put or write in the following:

#3. I am applying for one or more of the following:
* a work permit with the same employer * An initial work permit or work permit with a new employer
* restoration of your status as a worker * temporary resident permit

All of DETAILS OF INTENDED WORK IN CANADA

Between these two questionnaires, I'm not really sure what to put on #3 while the "details of work..." my plan is not to put anything or N/A. Am I right with this one?

Please help me. Need your good knowledge.

You should change the font colour in your post to all black. The yellow can't be read.

Hi scylla. Thanks. I change already. I wasn't knowing it's a light yellow. After posting it I went for some errands that's why I wasn't able to check it and only now.
