I need help, can't get form imm 5708 and i need to complete by thursday!

I have tried for over a week, different browsers, etc to get form imm 5708 to open so i can fill out and attach to other application form on 0- line.
It does not work...can someone tell me what to do, i need to have this completed by thursday.

OK i know what problem they have , it's not downloadable via CHROME ,,PLEASE USE IE 11 . here is the link:-
