I lost the notification email with instructions for additional documents!

I received an email stating I should submit my IELTS results within 120 days. I accidentally deleted it and now don't have any instructions on where to submit it, to whom I should address it to and what I should write on the envelope. I don't even know the number I was assigned to. Is there a call center number I can call and give any other data (like my name, birthday, day my application was received at NV?) so I can retrieve this info? Can I ask them to send the email again?

Please help. I'm doing my application on my own. Thank you.

You send it to the visa office processing your application, if you don't have your file no. (Bxxxxxxxxxxxxx) then you reference your name dob cob. on the covering letter with it.

I thought the file number was an absolute necessity. Its been 12 hours since i found out i lost my email and i'm a nervous wreck. imagine how the waiting time for their response will be like for me.

the covering letter would be the checklist with ielts results checked.
