I lost the notification email on where to submit my IELTS result!

I received an email stating I should submit my IELTS results within 120 days. I accidentally deleted it and now don't have any instructions on where to submit it, to whom I should address it to and what I should write on the envelope. I don't even know the number I was assigned to. Is there a call center number I can call and give any other data (like my name, birthday, day my application was received at NV?) so I can retrieve this info? Can I ask them to send the email again?

Please help. I'm doing my application on my own. Thank you. :'(
