I got the email from the CIC, and have some question

HI,

I got a email from the CIC, and they ask me to send something to them. This is the message what I got:

MISSING SCHEDULE A INFORMATION
If you have a copy of the application you submitted please review the application in particular the
schedule A Background information. Section 8 Personal History and Section 12 Addresses must cover the
10 year history prior to your submitting your application or beginning at the age of 18 whichever is the
most recent. If you left gaps in the dates and they do not cover the entire period send in a newly
completed Schedule A to provide complete information. If there are gaps in your personal history or gaps
in your address history your application will be delayed.
All family members over 18 must have a Schedule A with the above listed information.

So, I need to rewrite my schedule A to them? Is it only Section 8 and Section 12?

And

POLICE CERTIFICATES
Information and documents you need:
Completed application form
Your Hong Kong identity card or valid travel document
The original letter from the Canadian Consulate General requesting a police certificate
The original and a photocopy of the documents proving your relationship with the Principal Applicant for the visa application if the letter from the Canadian Consulate General does not contain your name (for dependents of the Principal Applicant only)
Fingerprints will be taken at the time of application
Applicable fees

I want to know how can I get "The original letter from the Canadian Consulate General requesting a police certificate"?
